Consultations

To make an appointment please call the rooms on (07) 3870 0116 Monday to Friday between 8.00am and 4.30pm.

  • We require your referral prior to your appointment. Please FAX it to our rooms on (07) 3870 1582. Dr Melsom will NOT see you without a referral
  • Please arrive 10 minutes prior to your appointment to complete paperwork
    Alternatively, fill in the New Patient Information Form and bring it along to your appointment. It can be downloaded and printed from here
  • Bring ALL x-rays, ultrasounds, MRI scans, nerve studies and other test results in relation to your injury
  • WorkCover patients MUST bring their claim number. If you do not have a claim number, you are responsible for paying your account on the day of consultation

Please note 24 Hours Notice is required for all cancellations or a Cancellation Fee (75% consultation fee) will apply

 

SMS Reminders – If you have given us your mobile phone number, we will send you an SMS reminder one day prior to your appointment. Please reply to this SMS to confirm your appointment. Any patients wishing to cancel must phone the rooms.

 

Payment Information
Payment is required at the time of your consultation. You may pay by Cash, Cheque, Credit Card (MasterCard and Visa only) or EFTPOS.

Please advise the staff prior to your consultation if you are requiring your account to be sent elsewhere i.e. WorkCover or a Third Party Insurance Company. You must provide your claim number, insurer contact and claim details, as enquiries will be made to confirm payment of your account prior to your consultation. If a third party does not accept liability you are responsible for your account and must pay at the time of your consultation.
